Radio buttons and "Don't know" check box doesn't align vertically

The “don’t align vertically” issue shows up because the don’t know is now with the other buttons, but it wasn’t there before. And to fix it, I’ll need to include the don’t-know with the rest of the buttons for multiple choice and all-that-apply, but keep it where it is for the free text and fill in the blank questions.
